Social Services Designee

Garden Park Care Center is looking for a compassionate, organized, and resident-focused Social Services Designee to join our team! If you're passionate about advocating for residents, supporting families, and helping create a positive long-term care experience, we'd love to hear from you. What You’ll Do Support residents and families throughout their stay Assist with admissions, discharges, and care planning Coordinate care conferences and communicate with interdisciplinary teams Connect residents and families with community resources Advocate for residents' rights and emotional well-being Maintain accurate documentation and ensure regulatory compliance Qualifications Long-term care or skilled nursing experience preferred Previous social services or healthcare experience preferred Strong communication and organizational skills Compassionate, professional, and resident-centered approach LSW a plus, but not required Why Garden Park?
