Key Responsibilities\n

• Utilize your education, training, and experiential knowledge to independently assess and formulate psychosocial diagnoses for hospice patients, ensuring tailored support strategies.

• Implement evidence-based behavioral interventions, including solution-focused treatment, motivational interviewing, and problem-solving therapy, to promote positive mental health outcomes.

• Engage in regular communication with interdisciplinary teams including psychologists, primary care providers, and additional healthcare practitioners to ensure a cohesive approach to care delivery.

• Methodically track and document patient progress toward treatment goals, adjusting strategies as necessary based on continual assessment and collaboration.

• Support psychotropic medication management as prescribed by medical providers, emphasizing adherence, evaluating side effects, and assessing overall treatment effectiveness.

Qualifications\n

Compensation and Benefits\n
