Work Environment

This position requires onsite work within the home care setting. You will travel to various patient locations across New Jersey, providing therapy in a familiar environment that promotes comfort and trust. The flexibility in scheduling allows you to choose your hours between Monday and Friday, giving you the ability to create a work-life balance that suits your needs.

Key Responsibilities

• Conduct thorough evaluations of patients in their homes, determining their needs and setting specific, measurable therapeutic goals.
• Develop and implement personalized treatment plans utilizing a diverse range of therapeutic techniques, including exercise regimens, stretching, manual therapy, and specialized equipment.
• Document treatment progress meticulously, maintaining accurate records of patient responses and outcomes to inform ongoing treatment strategies.
• Adjust treatment plans as necessary, based on patient progress and any changes in health status.
• Provide education and training for patients and caregivers on proper use of assistive devices, self-care techniques, and exercise programs.
• Engage in continuous professional development through workshops and conferences to maintain the highest level of clinical expertise.

Patient Population / Client Focus

This role focuses on a diverse patient population, including children, adults, and the elderly, each dealing with a variety of diagnoses ranging from post-operative recovery to chronic disease management. By working closely with your patients, you will play a pivotal role in helping them regain independence, improve mobility, and enhance their overall quality of life.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• Must hold a valid state license in Physical Therapy.
• Excellent verbal and written communication skills to effectively interact with patients and families.
• Strong organizational and time management skills to handle multiple patient schedules efficiently.
• Basic proficiency in word processing and electronic record-keeping systems.
• Highly adaptable, with the ability to handle the dynamic nature of home care.

Preferred Experience

• While prior work experience is not mandatory, candidates with experience in home health care or outpatient therapy settings will be highly regarded.
• Experience working with diverse patient populations, including pediatrics and geriatrics, is a plus.

Compensation and Benefits
