Position Overview

Lyrad Physical Therapy is seeking a dedicated and compassionate Licensed Physical Therapist to join our growing team in Matawan, New Jersey. This full-time position involves providing expert care in home settings, allowing patients to achieve their rehabilitation goals in a familiar and comfortable environment. Work Environment

This role is primarily focused on home care, allowing therapists to bring professional care directly to patients in their residences. The dynamic environment enables therapists to custom-tailor treatment plans based on individual patient needs, fostering a collaborative atmosphere among caregivers, family members, and health professionals. A typical work schedule includes Monday through Friday 40-hour shifts, promoting work-life balance.

Key Responsibilities

• Evaluate patients referred by physicians and craft comprehensive, individualized treatment plans to meet their rehabilitation needs.
• Implement therapy techniques, monitor patient progress, and adjust treatment protocols as necessary to optimize rehabilitation outcomes.
• Provide skilled interventions based on best practices to ensure sustainable improvement in patients' conditions and functional abilities.
• Educate patients and their caregivers on treatment goals, exercises, and at-home techniques to support ongoing recovery.
• Maintain thorough documentation of patient evaluations, progress notes, and discharge summaries in accordance with healthcare regulations.
• Collaborate with a multi-disciplinary team to enhance patient care strategies and ensure seamless service delivery.
• Demonstrate a commitment to patient satisfaction by providing excellent customer service and effectively addressing any concerns that may arise.

Patient Population / Client Focus

The successful candidate will work with a diverse patient population, addressing a wide array of specialties including orthopedic injuries, pediatric conditions, neurological disorders, and sports rehabilitation. Each therapist will engage actively with patients in their home settings, contextualizing therapy within the patient's daily life to enhance motivation and compliance.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution is required.
• Active state licensure as a Doctor of Physical Therapy by the Physical Therapy Examining Committee.
• A minimum of two (2) years of relevant experience in a clinical setting, ideally encompassing home care.
• Current CPR certification to ensure patient safety during all treatment sessions.
• Strong communication skills, both written and verbal, to effectively connect with patients and their families.
• Proficiency in basic computer skills for documentation and communication.
• A passion for building relationships and positively impacting patient lives.

Preferred Experience

• Experience working in a home health care setting, demonstrating an understanding of the unique challenges and joys of providing care in patients' homes.
• Knowledge of various therapeutic modalities and their application in different patient populations.
• Experience collaborating with multidisciplinary teams and understanding different healthcare roles is advantageous.
