Work Environment

As a Physical Therapist in our home care facility, you will have the unique opportunity to provide therapeutic services directly in patients' homes. This environment allows for a more personalized approach to treatment, as you will observe patients in their daily routines and adjust therapies accordingly. Working Monday to Friday, you will participate in 40-hour shifts, allowing you to maintain a balanced work-life schedule.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred to you by healthcare providers, assessing their physical abilities, limitations, and conditions.
• Design, implement, and modify individualized treatment programs that align with the physician's plan of care, focusing on proven therapeutic techniques.
• Utilize evidence-based practices in treatment sessions to maximize patient outcomes, ensuring that each therapy session is purposeful and goal-oriented.
• Document patient progress meticulously and communicate effectively with the healthcare team to coordinate the best possible care.
• Provide education and training to patients and their caregivers on therapeutic exercises, mobility strategies, and self-management techniques.
• Exhibit excellent customer service and contribute positively to the overall effectiveness and reputation of our clinic.
• Meet established productivity and utilization goals while actively participating in team meetings and continuing education opportunities.

Patient Population / Client Focus

In this role, you will work with a diverse patient population, including individuals experiencing orthopedic challenges, pediatric conditions, neurological disorders, and sports injuries. This variety ensures that you can apply your expertise across multiple specialties, enhancing your skills and keeping your daily work engaging. Building strong relationships with your patients is crucial, as your goal is to inspire them to exceed their functional objectives and maintain independence.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.
• Current State License to practice physical therapy in New Jersey.
• Basic Life Support Certification from the American Heart Association.
• Strong verbal and written communication skills, with proficiency in word processing and basic computer applications.
• Capability to establish rapport with patients, fostering trust and encouraging them towards their rehabilitation goals.
• A commitment to positive community engagement and relationship-building.

Preferred Experience

• A Doctorate of Physical Therapy (DPT) is preferred but not mandatory.
• Two (2) years of clinical experience, particularly in home care or outpatient settings, is highly desirable.
• Current CPR certification and knowledge of contemporary therapy techniques and frameworks.

Compensation and Benefits
