C.A.R.E Physical Therapy, PLLC is seeking a dedicated and dynamic Physical Therapist to join our team in Woodbridge, New Jersey. As a full-time staff member, you will deliver exemplary care and rehabilitation services to help patients regain mobility and enhance their quality of life through personalized treatment plans. Work Environment

This position is based onsite, primarily in our patients' homes in the Woodbridge area. Each day presents a variety of challenges and rewards as you visit patients in their familiar surroundings, delivering care that meets them where they are most comfortable. The work schedule is from 9 am to 5 pm, Monday to Friday, offering a consistent routine while allowing ample time to assess and assist each patient individual.

Key Responsibilities

• Conduct thorough assessments of patients' physical abilities and limitations, creating tailored rehabilitation plans based on their specific goals.
• Provide individualized treatment, employing manual therapy techniques and modalities designed to enhance mobility and recover function.
• Maintain a manageable caseload that allows for focused attention and consistent support for each patient.
• Utilize evidence-based practice techniques to develop and implement effective treatment strategies.
• Educate patients and their families on recovery processes, preventative care, and lifestyle modifications that support long-term wellness.
• Monitor patient progress meticulously and make adjustments to treatment plans as needed, ensuring optimal recovery outcomes.
• Utilize Electronic Medical Record (EMR) software, specifically WebPT, for accurate documentation of patient sessions and treatment plans.
• Collaborate with fellow healthcare professionals, including physicians, nurses, and occupational therapists, to create a cohesive care strategy.
• Promote physical wellness through the development of tailored fitness and wellness programs that cater to patients' recovery needs.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field from an accredited institution.

• Exceptional skills in developing and implementing effective exercise programs tailored to individual patient needs.
• Experience utilizing EMR systems, particularly WebPT, for thorough and precise patient documentation.

Preferred Experience

• A minimum of 2 years of relevant experience in outpatient physical therapy settings, specifically focused on orthopedic rehabilitation.
• Previous experience with home care delivery models is highly desirable, offering an understanding of the nuances of providing care in a home setting.

Compensation and Benefits
