Work Environment

Your role as a Full-Time Physical Therapist will take you directly into our patients' homes, allowing for personalized assessments and tailored therapeutic interventions. This onsite work setting means you can adapt your treatment strategies to the unique conditions of each home environment. Our supportive team is committed to fostering positive relationships with both our colleagues and the communities we serve, working Monday through Friday for a balanced work-life dynamic.

Key Responsibilities

• Conduct comprehensive evaluations of patients, including detailed history reviews and specific assessments to determine effective treatment plans.
• Communicate assessment findings and treatment strategies clearly to patients and their families, actively involving them in setting treatment goals.
• Implement tailored treatment protocols based on patient evaluations and continually assess their progress, adjusting interventions as necessary to reflect the evolving healthcare landscape.
• Collaborate with healthcare teams, recognizing critical changes in patient conditions that may require modifications to care plans or further consults.
• Ensure timely and accurate documentation of patient interactions, including follow-up care plans and discharge evaluations, emphasizing clarity and compliance with healthcare regulations.
• Manage your time effectively, meeting productivity expectations while maintaining an organized schedule and engaging in community outreach initiatives.
• Participate in departmental goal-setting and compliance with regulatory standards, embodying our commitment to excellence.
• Pursue continuous professional development opportunities and take on clinical instructor roles to mentor students and peers.
• Enhance the patient experience by remaining adaptable and responsive to their needs, while actively participating in initiatives to remove access barriers.

Patient Population / Client Focus

In this position, you will work with diverse patient populations, including those recovering from surgery, managing chronic conditions, or seeking to improve their overall physical health. The home care setting allows you to deeply understand each patient's individual circumstances, which enhances your ability to craft personalized therapy plans that align with their rehabilitation goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid state license to practice as a Physical Therapist in New Jersey.
• Certification in Basic Life Support (BLS) through the American Heart Association.
• Ability to demonstrate strong interpersonal communication skills and a commitment to compassionate care.

Preferred Experience

Compensation and Benefits
