Full-Time Licensed Physical Therapist for Home Care Services

Transformative Rehab & Physical Therapy is seeking a passionate and skilled Physical Therapist to join our dedicated team in Plainfield, New Jersey. Work Environment

Located in Plainfield, New Jersey, you will primarily conduct your therapy sessions in the homes of our clients. This unique setting allows you to implement customized treatment plans tailored to individual needs and daily life challenges. Your work schedule will be Monday through Friday, from 9 AM to 5 PM, providing a consistent routine to both you and the patients you serve.

Key Responsibilities

• Conduct thorough patient evaluations and assessments to understand their unique needs and set achievable therapeutic goals.
• Deliver one-on-one therapy sessions, managing a caseload of patients efficiently, with optimal time management to ensure each patient receives dedicated attention.
• Implement evidence-based treatment plans that focus on improving mobility, strength, and overall functional outcomes for patients.
• Collaborate closely with an interdisciplinary team, including physicians, occupational therapists, and nursing staff, to maximize patient care and address any emerging health concerns.
• Monitor and document patient progress continuously in compliance with facility protocols and healthcare standards, ensuring that treatment plans are dynamic and adjusted as needed.
• Educate patients and their families about their conditions and the importance of adherence to treatment regimens.

Patient Population / Client Focus

The ideal candidate will work with a diverse population, ranging from post-surgical patients to individuals with chronic conditions requiring rehabilitative care. Your expertise in orthopedic outpatient settings will be invaluable as you evaluate functional limitations and support clients in regaining their independence.

Qualifications

• Doctorate in Physical Therapy (DPT) from an accredited program.
• A valid license to practice as a Physical Therapist in the state of New Jersey.
• At least 2 years of experience in providing physical therapy, particularly in home care or outpatient settings.
• A clean record with no disciplinary actions against your physical therapy license.
• Demonstrated proficiency in manual therapy techniques and other therapeutic interventions.
• Exceptional communication skills, with the ability to build rapport with patients and collaborate with healthcare professionals.

Preferred Experience

• Experience in developing home exercise programs tailored specifically to individual recovery goals.
• Familiarity with electronic health record (EHR) systems for maintaining patient documentation.

Compensation and Benefits
