Work Environment

This position operates in a home care setting where you will work closely with patients in their homes. Key Responsibilities

• Conduct thorough examinations and evaluations for patients with orthopedic concerns, establishing accurate diagnoses.
• Develop and implement patient-specific treatment plans that align with best practices and recovery goals.
• Provide ongoing consultations with healthcare providers regarding patient care and progress throughout the treatment continuum.
• Maintain exceptional communication with patients and their families, ensuring they are informed and engaged in the therapeutic process.
• Document all assessments, treatments, and patient progress rigorously in compliance with healthcare regulations and standards.
• Participate in internal leadership functions, including community outreach, Continuous Quality Improvement (CQI), and program development.
• Supervise and mentor support staff, ensuring optimal patient care and effective utilization of resources.
• Engage in professional development through staff meetings, in-services, and continuing education programs.

Patient Population / Client Focus

The focus of our care is primarily on orthopedic patients, each with unique histories and challenges. You will be responsible for assessing a diverse range of clients, from post-surgical recovery patients to individuals managing chronic conditions. Working in patients' homes, you'll adapt treatment techniques to facilitate optimal recovery in a comfortable and familiar setting, essential for enhancing patient engagement and compliance.

Qualifications

• A valid Doctorate in Physical Therapy (DPT) from an accredited program is required.
• State licensure as a Physical Therapist and Basic Life Support (BLS) certification are mandatory.
• Proficiency in written and verbal communication and basic computer skills, including familiarity with documentation software.
• Demonstrated ability to foster collaborative relationships with patients, families, and multidisciplinary teams.

Preferred Experience

Previous experience in home care or outpatient orthopedic settings is highly desirable. Familiarity with treatment technologies and methodologies specific to orthopedic rehabilitation can enhance your effectiveness in this role. Strong organizational skills and the capacity to manage a varied caseload will be crucial for success.

Compensation and Benefits
