Work Environment

This role is onsite and operates primarily in a home care setting, allowing therapists to directly assess and treat patients in their own environments. Key Responsibilities

• Conduct thorough evaluations encompassing patient history, physical assessments, and evaluation findings to create effective, individualized treatment plans.
• Communicate evaluation results and treatment strategies to patients and their families, actively involving them in the goal-setting process.
• Implement treatment protocols effectively by adapting strategies based on evaluation outcomes and the patient's current health status.
• Collaborate with other healthcare professionals, documenting any changes in patients' conditions that require modifications to treatment plans or additional consultations.
• Prepare comprehensive discharge plans and ensure follow-up care, documenting all patient interactions, treatment progress, and evaluation results promptly and accurately.
• Exhibit strong time management skills to meet productivity expectations while maintaining an organized schedule for patient appointments and meetings.
• Contribute to the achievement of departmental goals by participating in both clinical and community outreach programs that promote patient access to care.
• Demonstrate a commitment to professional development through continuous education and mentorship, including acting as a clinical instructor for students.
• Promote a positive patient experience by being adaptable to their needs and actively seeking to streamline access to care.

Patient Population / Client Focus

Our physical therapists primarily serve clients with orthopedic conditions, including post-surgical rehabilitation, sports injuries, and chronic pain management. Qualifications

• Possession of a Doctorate in Physical Therapy from an accredited institution.
• A Bachelor's degree is preferred, alongside a valid High School Diploma or GED.
• A current state physical therapy license is required for the locations in which you will practice.
• Certification in Basic Life Support (BLS) from the American Heart Association for Healthcare Providers is necessary.
• While prior work experience is not required, a passion for helping others and strong interpersonal skills are essential.

Preferred Experience

• Experience in orthopedic rehabilitation or outpatient settings is a plus.
• Familiarity with electronic health record (EHR) systems to document patient care accurately.
• Adequate knowledge of current best practices in physical therapy and ongoing developments in rehabilitation sciences.
