Work Environment

As an onsite physical therapist working in a home care setting, your daily responsibilities will take place in patients' homes, allowing for personalized care tailored to their unique environments. You will have the flexibility to create a therapeutic atmosphere that promotes healing and recovery while maintaining compliance with healthcare regulations. Reporting to the Clinical Director, you will work closely with a supportive team of healthcare professionals dedicated to long-term patient wellness and satisfaction.

Key Responsibilities

• Conduct thorough evaluations including the review of patient history and specific assessments to determine treatment needs and set attainable goals.
• Develop and explain customized treatment plans, ensuring patients and their families are actively involved in goal setting and understanding the proposed interventions.
• Implement treatment protocols that are responsive to patient assessments, adapting your approach based on patients' evolving health conditions.
• Coordinate care with other healthcare professionals, recognizing changes in patient status that may necessitate modifications in the treatment plan.
• Document patient care activities accurately and promptly, creating a comprehensive record that reflects the patient's treatment journey and progress.
• Engage in discharge planning and follow-up consultations to ensure continuity of care to optimize recovery.
• Participate in community-building and marketing efforts to promote the clinic's services and enhance patient access to care.
• Maintain effective time management, meet productivity expectations, and attend to scheduling duties efficiently.
• Contribute to departmental goals, ensuring compliance with all relevant regulations and standards.

Patient Population / Client Focus

You will serve a diverse patient population, primarily focusing on orthopedic conditions ranging from post-operative rehabilitation to chronic pain management. Your approach will be holistic and centered on improving patients' quality of life while building strong therapeutic relationships based on trust and empathy.

Qualifications

• A Bachelor's degree in Physical Therapy (Doctorate in Physical Therapy preferred).
• A valid state license to practice physical therapy in New Jersey.
• Certification in BLS for Healthcare Providers from the American Heart Association is required.
• Strong analytical and communication skills to convey complex information effectively to patients and families.

Preferred Experience

Compensation and Benefits
