Key Responsibilities

• Conduct comprehensive patient evaluations, integrating historical information, specific assessments, and clinical judgments to develop tailored treatment plans.
• Effectively communicate evaluation findings and treatment options to patients and their families, encouraging their involvement in goal-setting processes.
• Implement evidence-based treatment protocols adjusted to individual patient presentations and progress, ensuring alignment with current healthcare practices.
• Collaborate with multidisciplinary teams to assess and document changes in patient conditions, modifying care plans as necessary.
• Engage in thorough discharge planning to ensure patients receive seamless follow-up care post-therapy, maintaining accurate and timely documentation throughout treatment paths.

Patient Population / Client Focus

As an Orthopedic Physical Therapist at Active Life, you will work primarily with patients recovering from orthopedic surgeries, injuries, or chronic conditions. Our emphasis lies in personalized rehabilitation strategies catering to a varied clientele, including athletes, post-operative patients, and individuals with degenerative disorders. Your approach will be holistic, addressing not just physical rehabilitation but also the emotional and psychological aspects of recovery.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Valid state license as a Doctor of Physical Therapist (DPT) held in New Jersey.
• A minimum of two (2) years of clinical experience in physical therapy, particularly in orthopedic settings.
• Current CPR certification, demonstrating commitment to patient safety.

Preferred Experience

• Experience in home care settings, providing direct therapy services in patients' residences.
• Background in pediatrics or geriatrics is considered an asset, expanding the diversity of care provided.
• Familiarity with electronic documentation systems to maintain compliance and enhance documentation efforts.
