Work Environment

This role is primarily focused on home care settings, allowing you to provide personalized therapy in the comfort of each patient's home. As an onsite position, you will travel to patients' residences, ensuring a direct and impactful therapeutic presence. Key Responsibilities

• Evaluate, plan, and implement individualized treatment programs for patients referred by physicians, ensuring that each plan aligns with evidence-based practices.
• Deliver specialized treatments for diverse clinical conditions, including orthopedic injuries, pediatric conditions, neurological disorders, and sports medicine cases.
• Provide high-quality customer service and maintain effective communication with patients, families, and the interdisciplinary healthcare team to optimize care delivery.
• Engage in ongoing assessments of patient progress and modify treatment plans as needed to address evolving patient needs and objectives.
• Educate and consult with patients and their caregivers to foster a strong support system for rehabilitation.
• Meet performance metrics and organizational objectives to ensure efficient service delivery and optimal patient outcomes.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.

• Current CPR certification to ensure the safety and well-being of patients.
