Work Environment

This position will primarily be conducted in the patients' homes, providing a unique and personalized setting for therapy sessions. As part of our on-site care model, you will work independently in various locations, creating a flexible work schedule that allows you to focus on each patient's needs in their own environment. Key Responsibilities

• Conduct comprehensive evaluations to determine patient needs and establish individualized treatment plans in collaboration with referring physicians.
• Implement effective therapeutic exercises and modalities aimed at alleviating symptoms, restoring function, and improving overall quality of life.
• Provide skilled and compassionate care by using evidence-based practices to maximize patient outcomes.
• Demonstrate exceptional customer service skills that foster positive patient interactions and build trust.
• Educate patients and their families about the rehabilitation process, exercises, and preventative measures.
• Maintain accurate and timely patient documentation in accordance with regulatory standards and organizational policies.
• Participate in team meetings and collaborate with multi-disciplinary teams to enhance patient care plans and address any challenges.
• Meet performance-based goals while actively contributing to the growth and improvement of services offered by Lyrad Physical Therapy.

Patient Population / Client Focus

You will engage with a diverse patient demographic, including individuals undergoing orthopedic, pediatric, neurologic, and sports-related rehabilitation. Your role will involve tailoring interventions to the unique circumstances of each client, enabling them to regain mobility and functionality in a supportive atmosphere.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Active state license as a Doctor of Physical Therapy (DPT) by the Physical Therapy Examining Committee.
• Minimum of two (2) years of relevant clinical experience in a similar setting.
• Current CPR certification and proficiency in basic life support methods.
• Strong communication skills, both written and verbal, essential for patient interactions and multidisciplinary collaboration.
• Proficient with computer applications and documentation software for maintaining patient records.
• Ability to build strong, trusting relationships with patients and their caregivers, fostering a supportive environment for recovery.

Preferred Experience

• Experience working in home health or similar settings, providing therapy in patients' residences.
• Familiarity with a variety of treatment modalities, including advanced techniques in manual therapy or specialized rehabilitation.
• Demonstrated commitment to lifelong learning and professional development.

Compensation and Benefits
