High Energy Body & Home Care is actively seeking a dedicated and compassionate Physical Therapist to join our team as we provide exceptional home health services in South River, New Jersey. This full-time, onsite role involves working directly with clients in their homes, delivering personalized and effective physical therapy treatments to maximize their functional abilities and enhance their overall quality of life.

Work Environment

As a Physical Therapist with High Energy Body & Home Care, your work environment will predominantly take place in patients' homes throughout South River and surrounding areas. This dynamic setting allows you to build meaningful relationships with clients while making a real impact on their rehabilitation outcomes. The collaborative environment includes regular communication with an interdisciplinary team, ensuring comprehensive care tailored to each client's unique health journey.

Key Responsibilities

• Direct and implement physical therapy treatments aimed at preventing disabilities, alleviating pain, enhancing muscle function, and maintaining optimal performance as directed in the care plan.
• Evaluate clients' functional capabilities through diagnostic assessments and develop individualized treatment plans.
• Conduct home evaluations to identify environmental modifications that can foster greater independence and functionality for clients.
• Administer prescribed therapy services, including manual therapeutic exercises, traction, and massage, while instructing clients in non-manual exercises and functional activities.
• Evaluate, fit, and recommend necessary adjustments for prosthetic and orthotic devices in collaboration with clients and physicians.
• Document and assess patient progress through comprehensive clinical records, including initial evaluations, progress notes, and discharge summaries.
• Coordinate with treatment teams during case conferences to ensure that the total treatment approach remains aligned with the best patient outcomes.
• Provide education and support to clients and their families, ensuring they understand the physical therapy plan and any assistive devices used during treatment.
• Supervise and guide Physical Therapy Assistants in delivering specific phases of care as per state regulations.
• Maintain timely submissions of clinical documentation and adhere to organizational policies.

Patient Population / Client Focus

Your primary focus will be on a diverse client population in South River, New Jersey, including those recovering from surgeries, managing chronic illnesses, or rehabilitating from injuries. Each client will benefit from individualized assessments and specialized interventions designed to optimize their recovery and promote independence in daily activities.

Qualifications

• Possession of a Bachelor's degree, with a preference for candidates who hold a Doctorate in Physical Therapy (DPT) from an accredited program.
• A minimum of one (1) year of experience as a physical therapist, preferably in a home care or acute care setting.
• Current, active state licensure in physical therapy.
• Certifications in CPR and a solid understanding of state and federal regulations pertinent to physical therapy.
• A proven ability to engage with clients and families, enhancing their understanding and participation in their care.
• Strong communication and interpersonal skills as well as a high commitment to quality and integrity in patient care.
• A positive attitude, adaptability, and a collaborative spirit in a team-oriented environment.

Preferred Experience

• Experience working in home health care settings is strongly preferred to foster a greater understanding of patient needs in a home environment.
• Familiarity with OASIS documentation and care planning processes.
• Demonstrated involvement in interdisciplinary teams and case conferences.

Compensation and Benefits

We offer a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
