Work Environment

This position is strictly onsite and oriented towards home care, allowing therapists to engage directly with patients in their own environments. Key Responsibilities

• Conduct comprehensive evaluations of patients in their homes to determine physical functionality and therapy needs.
• Develop and implement individualized treatment plans that incorporate varied techniques such as therapeutic exercises, manual therapy, and use of assistive devices.
• Monitor patient progress meticulously, adjusting treatment plans as necessary to meet evolving therapeutic goals.
• Educate patients and their families—teaching them effective techniques for managing their conditions and utilizing prescribed assistive devices.
• Maintain detailed documentation of patient progress, adherence to treatment plans, and overall effectiveness of therapies, in compliance with regulatory standards.
• Participate in continuous professional development and training through workshops and peer networking to stay current with advancements in physical therapy.
• Collaborate with multidisciplinary teams, including occupational therapists, nurses, and physicians, to ensure cohesive patient care.

Patient Population / Client Focus

You will be working with a diverse patient population ranging from children to elderly adults, each facing distinct challenges and recovery goals. Qualifications

• A Bachelor's Degree in Physical Therapy is required; candidates with a Doctorate of Physical Therapy (DPT) are preferred.
• Active State License in Physical Therapy, ensuring compliance with local regulations.

• Proficiency in word processing and computer applications for documentation and reporting.
• Two years of professional experience as a Physical Therapist, providing you with a robust foundation in patient assessment and therapeutic intervention.
• Current CPR Certification to ensure patient safety and readiness for emergencies.

Preferred Experience
