Texas Licensed Home Health Coordinator

Virtual Vocations Inc

To support patient care coordination, the part-time Texas Licensed Home Health Coordinator will manage new referrals, document workflow tasks in the electronic medical record, and collaborate with healthcare professionals while working remotely. Key responsibilities Receive and document new referrals in a timely manner Complete assigned workflow tasks in the electronic medical record Coordinate care with nurses, social workers, and caregivers Required qualifications Graduate of an accredited school of Vocational/Practical nursing preferred; high school diploma or equivalent required for non-clinical candidates One year of experience as a Licensed Vocational/Practical Nurse, preferably in homecare Current driver's license
