Compensation: $90,000-$130,000 per year , depending on experience

Schedule: Full-Time | Weekend & On-Call Rotation
Location: Greater Twin Cities Metro Area | 21-County Service Area (territories assigned based on coverage needs)

What You'll Do
- Manage your own hospice caseload while serving as the primary RN for patients and families.
- Complete comprehensive nursing assessments and develop individualized plans of care.
- Provide expert pain and symptom management focused on comfort, dignity, and quality of life.
- Guide and educate patients, caregivers, and families throughout the hospice journey.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.
- Provide care in patients' homes, assisted living communities, and skilled nursing facilities.
- Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.
- Participate in a weekend and on-call rotation to provide exceptional continuity of care.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Experience in hospice preferred , or a strong background in oncology, ICU, palliative care, home health, or other complex care settings.
- Strong clinical judgment and the ability to work independently.
- Excellent communication, organization, and critical thinking skills.
- A passion for relationship-centered, patient-focused care.
- Reliable transportation and willingness to travel throughout your assigned territory.
