Case Manager

RUTHS HOUSE OF HOPE

The Case Manager provides comprehensive case management services for Ruth’s House (RH), an emergency and transitional shelter serving up to nine families, and Sarah’s House (SH), a recovery residence serving up to five women. The Case Manager supports residents across both programs and helps create safe, stable, and supportive environments that promote long-term housing, recovery, independence, and personal well-being. Using a trauma-informed, strengths-based, and person-centered approach, the Case Manager works collaboratively with residents to develop individualized service plans, identify barriers, establish goals, and implement strategies that support stability and effective crisis management. The Case Manager serves as a primary resource and advocate for residents and helps connect them with appropriate community resources and services. This is a highly relational position requiring strong communication, networking, advocacy, and collaboration skills. The Case Manager develops and maintains partnerships with local service providers, treatment facilities, hospitals, government agencies, and other community resources throughout Minnesota to support residents and promote awareness of RH and SH programs. The Case Manager is a member of the Leadership Team, reports directly to the Program Director, and participates in shared organizational leadership and decision-making. Key Responsibilities and Duties: Resident Case Management & Support Admissions, Documentation & Program Compliance Follow established admissions and intake procedures for both RH and SH programs. Complete intake and required documentation in the case management software (CMS). Follow GRH funding and transfer procedures and submit required information to the County in a timely manner. Communicate proactively with county and government agencies regarding resident benefits, including Housing Support and other assistance programs. Monitor deadlines and ensure required forms and documentation are complete and submitted accurately and on time. Adhere to all program policies, procedures, resident agreements, and expectations. Coordinate with the Program Director when significant policy violations or concerns arise. The Program Director determines when a resident is asked to leave programming, and the Case Manager will participate in reviewing the situation and developing an appropriate transition plan. Boundaries & Ethics: Maintains strong professional boundaries, confidentiality, ethical standards, and appropriate role clarity. Crisis Management: Remains calm and effective during stressful or crisis situations and uses appropriate de-escalation and problem-solving strategies. Organization & Prioritization: Effectively manages multiple responsibilities, deadlines, residents, and program needs while maintaining accurate documentation. Collaboration: Works cooperatively with residents, coworkers, leadership, agencies, and community partners toward shared goals. Adaptability: Responds appropriately to changing circumstances and balances priorities between two residential programs. Professionalism: Represents the organization positively and responds constructively to supervision, feedback, and opportunities for growth. Qualifications & Requirements Bachelor’s degree in social work, psychology, human services, or a related field preferred; equivalent relevant education and experience may be considered. Experience working with people experiencing homelessness, housing instability, recovery, or other complex life circumstances preferred. Experience working with diverse populations and demonstrated ability to provide respectful, inclusive services. Demonstrated ability to work independently while also functioning effectively as part of a team. Strong communication and interpersonal skills. Ability to provide trauma-informed care while maintaining accountability to individualized service plans and program expectations. Strong organizational skills and ability to prioritize competing responsibilities. Ability to receive and respond professionally to constructive feedback. Understanding of confidentiality requirements and the ability to maintain confidential resident information. Availability to work at least one evening shift per week, holidays as scheduled, and two weekend shifts per month. Availability to participate in the rotating RH on-call schedule and respond to emergency situations as required. Valid driver’s license and proof of automobile insurance. Proficiency with Google Workspace, including Gmail, Docs, and Sheets, as well as case management software and other relevant technology; ability and willingness to learn new software. Ability to pass required background checks and maintain eligibility for employment. Ability to pass the required NETstudy 2.0 background study and maintain eligibility. Must complete NARR certification within 45 days of hire. Ability to lift approximately 20–25 pounds. Ability to climb stairs and perform the physical requirements of working in residential program environments. Insurance stipend Paid Time Off (PTO) Work Location & Schedule Work Location: In person at Ruth’s House and Sarah’s House, with occasional work at the Outreach Office. The position requires evening, weekend, holiday, and rotating on-call availability as described above.
