Shift: 12H Days 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday

Job Description:

The Social Worker is responsible for acute inpatient medical case management, including discharge planning within a large hospital setting. Candidates must have experience in a high-volume environment and should be able to adapt quickly with minimal training.

Responsibilities:

• Manage a caseload of 15-20 patients per day.
• Coordinate acute care and ED discharge planning.
• Consult with Respiratory Therapy and Pharmacy for discharge planning as needed.
• Participate in a rotating schedule including some weekends.

Requirements:

• ACTIVE License - LMSW or LCSW and ACTIVE MS State License.
• Minimum 2 years of Acute Inpatient Medical Case Management experience including 1 year in a large hospital.
• Proficiency in EPIC charting.
• Previous travel experience in a level 1 facility within the last 3 years.
• BLS Certification.
