Key Responsibilities\n

As a Licensed Clinical Social Worker at Sunny Valley, you will be entrusted with a variety of responsibilities that are essential for the recovery journey of our clients. Your specific duties will include:

• Conducting comprehensive assessments to determine the counseling needs of clients, including thorough evaluations for substance use disorders based on DSM criteria.

• Collaborating with multidisciplinary teams to create and implement individualized treatment plans that align with American Society of Addiction Medicine (ASAM) criteria.

• Facilitating individual therapy, group therapy, and family sessions, utilizing evidence-based therapeutic practices to promote recovery.

• Documenting all aspects of client interactions, assessments, and treatment progress in their clinical records, ensuring compliance with best practices and standards.

• Development and adjustment of discharge and continuum of care plans to guide clients into appropriate follow-up services.

• Maintaining a manageable client caseload while ensuring thorough case management and utilization review updates.

• Coordinating support services for clients and their families, initiating contact with referral sources, including legal, educational, and workplace representatives, as needed.
