Licensed Practical Nurse (LPN) - Nursing Facility

Assignment Duration

This is a 13-week contract position.

Schedule

Mid shift, 5x8-hour schedule from 3:00 PM to 11:30 PM. Weekend coverage required every other weekend.

Job Description

The Licensed Practical Nurse (LPN) will provide nursing care to patients in a nursing facility. Responsibilities include administering medications, monitoring vital signs, and assisting with daily living activities. The LPN will collaborate with registered nurses and healthcare professionals to deliver high-quality patient care.

Responsibilities

• Monitor and record patient vital signs and report any significant changes to the healthcare team.
• Administer medications and treatments as prescribed by healthcare providers.
• Assist patients with daily living activities, ensuring comfort and dignity.
• Document patient care activities accurately using the EPIC charting system.
• Ensure compliance with safety and infection control protocols.

Required Experience / Certifications / Licensure

• Active New York Single State LPN license required; no pending licenses accepted.
• BLS certification is required.
• Minimum of 1 year of recent nursing experience in a long-term acute care setting.
• Experience with EPIC charting is mandatory.
• Must be able to float within skill set as required.
