Outreach Nurse

WORKSITE LOCATION: GBC Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904 STARTING PAY: $18.69 EEOC CODE: Professional FLSA STATUS: Non-Exempt SHIFT: Fulltime; Hours may vary based on customer needs. JOB PURPOSE: The Outreach Nurse is a Licensed Vocational Nurse (LVN) capable of practicing in a person-centered, family-centered, and trauma-informed manner. The Outreach Nurse will work closely with the other members of the interdisciplinary treatment team to achieve a wholistic approach to care. DEGREE AND/OR CERTIFICATION: Required: Graduation from an accredited school of nursing; a current, uninfringed license to practice vocational nursing (LVN) in the State of Texas; at least three years' experience in nursing. Preferred: Hospital or home health experience. 2.Providequality services to customers in a community setting with minimal supervision. 3.Conducta comprehensive assessment of the customers' environment, physical and mentalwellbeing. 4.Administerpsychotropic medications as prescribed, monitor for adverse reactions. 5.Gainand maintain knowledge of proper AIMS assessment technique with assessmentcompletion no less frequently than every 90 days for applicable customers. 6.Gainand maintain knowledge of venipuncture performance. Monitor customers foroutstanding lab orders and complete as needed. 7.Providepill packing, education and supervision to customers as required. 8.Meetsrequire productivity of 60 hours of face-to-face contact each month. 9.Collaboratewith all members of the person-centered treatment team to ensure best possiblecustomer outcomes. 10. May perform other duties asassigned by the Director of Nursing Services. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
