Work Environment

This position is primarily onsite, allowing for direct interaction with patients in a community-focused setting. Our typical work schedule runs from 9:00 a.m. to 6:00 p.m., with some occasions requiring participation in evening clinics until 8:00 p.m. Additionally, you may engage with the community during weekend health fairs or outreach events. The average workweek is approximately 40 hours, ensuring a healthy work-life balance while providing essential care.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, gathering critical information to develop effective treatment plans.
• Diagnose and manage a range of acute and chronic medical conditions, emphasizing preventive care and health promotion strategies.
• Lead collaborations with community organizations to design and implement health education programs tailored to the specific needs of the population.
• Deliver culturally sensitive care while advocating for patients’ rights and ensuring they can access the necessary healthcare services.
• Coordinate referrals and follow-up care to guarantee continuity and quality of care for all patients.
• Accurately maintain detailed medical records in compliance with regulatory standards, utilizing electronic health record (EHR) systems to document interactions.

Qualifications

• Current Nurse Practitioner certification.
• Fluency in Spanish is required to effectively communicate with the patient demographic.

Compensation and Benefits
