• Weekend Requirement: Every other weekend

Requirements

• Active California RN License
• 2+ years of current Acute Care Hospital Case Management experience (Care Coordination & Discharge Planning required)
• Epic experience
• Previous travel experience preferred (strong first-time travelers may be considered)
• Experience with HMOs/IPAs, Advance Directives, POLST, and Protective Services
