Job Description​🌟 We're Hiring: Advanced Practice Provider – Palliative Care (Full-Time Days)Join our compassionate and collaborative Palliative Care Team and make a meaningful difference in the lives of patients and their families.Position DetailsFull-Time Day ShiftSchedule: Monday–Friday, 8:00 AM – 5:00 PMPrimary Location: Saint Luke's PlazaOpportunity to float to additional sites as neededAbout the RoleAs an Advanced Practice Provider (APP), you will provide advanced clinical care to assigned patient populations while serving as a key member of an interdisciplinary team. Responsibilities include:Assessing, collecting, and analyzing patient health dataDeveloping, implementing, and evaluating individualized plans of careCoordinating comprehensive patient care and case management servicesProviding expert clinical consultationUtilizing prescriptive authority, procedures, and treatments in accordance with state and federal regulationsEducating patients, families, and healthcare staffManaging high-risk patient populationsSupporting quality improvement and research initiativesPromoting positive patient outcomes and enhancing care delivery systemsCall, Weekend & Holiday RequirementsCall coverage: One week (7 consecutive days) at a timeCurrently, call and weekend coverage are provided only at Saint Luke's PlazaCall and weekend commitment: Approximately 1–2 times per quarterHoliday commitment: One holiday per yearWhy Join Our Team?At Saint Luke's, you'll work alongside dedicated clinicians who are committed to providing exceptional palliative care that improves quality of life for patients and their families.
