Work Environment\n

This full-time role allows for a remote work setting, supporting our Hospice Center's efforts to provide exceptional care without geographical constraints. The work schedule typically runs Monday to Friday from 8:00 AM to 4:30 PM, although flexibility is essential to accommodate client needs, including potential evening or weekend visits when necessary. On-call duties may arise to address urgent situations, ensuring continuous support for our clients.

Key Responsibilities\n

• Conduct in-depth assessments of seniors’ psychosocial needs, addressing emotional, mental, and social factors to identify individual challenges.

• Work collaboratively with clients and their families to devise tailored care plans that reflect personal goals, preferences, and cultural considerations.

• Provide ongoing counseling and emotional support as clients navigate significant life transitions, such as health crises or changes in living arrangements.

• Advocate for clients’ rights to accessing community resources and services, ensuring they receive the comprehensive support they need.

• Engage with interdisciplinary teams, including nursing staff and healthcare professionals, to ensure a cohesive and enhanced care approach.

• Monitor clients’ well-being through regular visits, assessing living conditions and proactively addressing any safety concerns.

• Foster communication among family members involved in client care, mediating conflicts and promoting cohesive support strategies.

• Maintain meticulous documentation of all client assessments, care plans, and interactions in compliance with ethical and legal standards.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid Licensure as a Licensed Clinical Social Worker (LCSW) in the state of Connecticut.

Compensation and Benefits\n

The compensation range for this role is competitive, with an annual salary between USD 60,000 and 100,000, commensurate with experience and qualifications.
