King Manor Care Center is seeking a Social Worker to plan and administer social services for residents, coordinating discharge planning and community resources. The role involves working with the Director of Social Services and other departments to ensure high‑quality care. Responsibilities include counseling, maintaining records, and liaising with families, outside agencies, and staff.
