Community Health Navigator & Diversion Specialist

Independent Living, Inc. is seeking a Social Care Network Navigator & Diversion Specialist to connect individuals with health-related social needs to community resources. This role works in hospital and community settings, performing screenings, eligibility determinations, and coordinated referrals to improve health outcomes.
