Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children and families within the hospice context.

• Engage actively in the development and implementation of Individualized Family Service Plans (IFSPs) tailored to each child’s specific needs.

• Monitor and analyze outcomes as part of a collaborative team to ensure effective service delivery.

• Provide supportive resources, skills, and information to families, empowering them to actively participate in their child's developmental journey.

• Comply with all applicable federal and state regulations to maintain high standards of care.

• Employ evidence-based practices in developing joint plans with families, emphasizing the benefits of a natural learning environment.

• Utilize the coaching model and natural environment teaching to enhance learning experiences for children.

• Ensure the health and safety of all staff members and individuals served by adhering to organizational policies and procedures.

Patient Population / Client Focus\n

In this role, you will primarily work with families of children facing developmental delays or disabilities within a hospice care framework. You will engage with a diverse population, offering culturally competent care that respects and honors each family's values and experiences. Through targeted assessments and personalized interventions, you will significantly contribute to improving developmental outcomes for these children and their families.

Qualifications\n

• A Master's Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) issued by the Department of Public Health.

• A valid driver's license to facilitate home visits as needed.

Compensation and Benefits\n
