Key Responsibilities\n

• Maintain accurate and timely documentation, including clinical and progress notes, in compliance with department policies to ensure quality care.

• Collaborate closely with members of a multidisciplinary healthcare team to develop effective treatment plans and discharge plans based on thorough psychosocial assessments.

• Engage and consult with families and support systems to coordinate treatment and ensure a seamless transition to discharge planning.
\n
• Create tailored, individualized treatment plans that are responsive to patient needs, reflecting cultural sensitivity and clinical best practices.

• Conduct regular counseling sessions with clients, continuously evaluating and adjusting treatment plans to meet evolving needs and circumstances.

• Implement strategies for ongoing assessment of patient progress and report findings during team meetings to enhance collective understanding and care integration.

Patient Population / Client Focus\n

You will primarily work with school-aged children and adolescents facing various mental health challenges, including anxiety, depression, behavioral issues, and trauma-related concerns. Qualifications\n

• A Master’s degree in Social Work, Counseling, or a related field is required.

• Must possess a valid Licensed Clinical Social Worker (LCSW) certification in the state of Pennsylvania.
