Responsibilities:

• Provide direct patient care to our residents, including administering medications, performing vital signs, and assisting with daily living activities
• Develop and implement individualized care plans in collaboration with our interdisciplinary care team
• Conduct regular assessments to monitor patient progress and adjust care plans as needed
• Communicate effectively with patients, families, and colleagues to ensure seamless transitions and optimal care outcomes

Requirements:

• Graduate of an accredited Licensed Practical Nurse (LPN) program
• Current LPN licensure in the state of Pennsylvania
• Ability to work varied shifts, including days, evenings, and weekends
