Field-Based Lead Care Manager — ECM & Community Health
BLEHEALTH, LLC
BLEHEALTH, LLC in the Los Angeles metro area seeks a Lead Care Manager to coordinate care for chronically ill and high-risk members within the ECM program. You will work with members, families, providers, and community resources to implement individualized care plans and ensure timely access to medical, behavioral health, and social supports.
