RN Care Coordinator -Radiation Oncology

As an RN Care Coordinator for the Radiation Oncology department at Cleveland Clinic, you will collaborate with a multidisciplinary care team to provide coordination of care and disease management for high-risk patients. This role focuses on patient outreach and care coordination to achieve optimal outcomes and promote wellness. Highlights Pay: $63,250.00 - $96,467.50 Location: Cleveland, OH Facility: Cleveland Clinic Main Campus Department/Unit: Radiation Oncology Schedule: Full-time, Day shift What You'll Do Work collaboratively with a multidisciplinary care team to develop goals and plan interventions. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments addressing various patient needs. Inform and work with patients and families regarding care coordination. Monitor patient compliance with care plans and perform reassessments. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Identify barriers, facilitate solutions, and connect to community resources.
