Job Description
The Homecare Occupational Therapist plays a vital role in delivering personalized therapy services to patients within their home environment. Working as part of a multidisciplinary team, this position involves daily home visits to assess patient needs, develop rehabilitation plans, and coordinate care to promote independence and improve quality of life.
Responsibilities
- Conduct thorough patient assessments to evaluate physical and cognitive abilities
- Perform daily home visits to deliver occupational therapy services
- Coordinate care with multidisciplinary team members to ensure comprehensive support
- Document patient progress and update rehabilitation plans accordingly
Preferred Qualifications
- Entry-level experience in occupational therapy
- Master's degree in Occupational Therapy
- Licensed Occupational Therapist
- Proficient in patient assessment and rehabilitation planning
