The Homecare Physical Therapist delivers personalized physical therapy services through individual home visits, focusing on managing chronic conditions and treating new patients. This role involves daily patient visits and operates within a care delivery model that emphasizes one-on-one attention, reporting directly to a clinical supervisor to ensure high-quality outcomes and coordinated care.
Responsibilities
- Provide physical therapy services through daily individual home visits
- Assess and manage chronic conditions with tailored treatment plans
- Develop and implement therapy programs for new patients
- Monitor patient progress and adjust treatments as necessary
- Coordinate care and report to the clinical supervisor regularly
- Educate patients and caregivers on therapeutic exercises and injury prevention
Preferred Qualifications
- Entry-level experience in physical therapy
- Master’s degree in physical therapy
- Licensed Physical Therapist
