Key Responsibilities

• Oversee discharge planning and ensure follow-up care, meticulously documenting all patient interactions to create a comprehensive history of care, including treatment plans, daily notes, progress summaries, and discharge evaluations.
• Manage your time effectively, ensuring punctuality for appointments and meetings, and actively participate in community outreach and marketing activities to enhance patient access and awareness.
• Contribute to departmental goals by staying updated on governmental regulations and compliance standards, while also contributing to a culture of excellence.
• Pursue continuing education opportunities for personal and team growth, serving as a clinical instructor for students in the field.

Patient Population / Client Focus

Qualifications
