Work Environment

As a Physical Therapist at Lifepoint Physical Therapy, you will work onsite in a state-of-the-art rehabilitation facility located in Jacksonville, FL. This full-time position offers 40 hours per week with variable weekends to accommodate patient needs. Key Responsibilities

In your role, you will:

• Conduct thorough examinations and evaluations of patients to identify functional limitations and develop tailored treatment plans.

• Provide consultation and collaborate with healthcare providers regarding each patient's continuum of care, ensuring seamless communication and coordination.
• Maintain accurate and comprehensive documentation of patient evaluations, treatment sessions, and discharge plans, adhering to all regulatory and professional standards.
• Engage patients, families, and caregivers in the treatment process, offering education and support to enhance rehabilitation outcomes.

Qualifications

Compensation and Benefits
