This is an exciting opportunity for a dynamic professional who thrives on building meaningful partnerships and driving growth in a collaborative, mission-driven environment./ph3Position Summary/h3pThe Hospice Clinical Liaison is responsible for growing patient census while serving as a positive ambassador for our hospice program within the community. This role develops and implements structured outreach strategies to cultivate new referral sources and maintain strong, ongoing relationships with physicians, hospitals, skilled nursing facilities, and other key partners./ppThe ideal candidate is solutions-oriented, proactive, and excels at relationship-building, follow-through, and collaboration throughout the referral-to-admission process./ph3Qualifications Skills/h3ulliLVN, RN, or MSW required/lili12 years of healthcare marketing/sales experience preferred/liliMinimum of 2 years hospice experience highly preferred/liliStrong familiarity with local referral partners including:/liulliHospitals/liliPhysicians/liliBoard Care (BC)/liliAssisted Living Facilities (ALF)/liliResidential Care Facilities for the Elderly (RCFE)/liliSkilled Nursing Facilities (SNF)/li/ulliExcellent written and verbal communication skills/liliSelf-directed with the ability to work independently/liliDetermined, collaborative, and strong work ethic/liliConsistent, persistent, patient, and compassionate interpersonal approach/liliValid drivers license, clean driving record, and current auto insurance required/li/ulh3Key Responsibilities/h3ulliBuild and maintain relationships with physicians, hospitals, skilled nursing facilities, discharge planners, and community partners/liliPlan and coordinate marketing events, educational sessions, and community luncheons/liliEducate healthcare stakeholders on hospice and palliative care services/liliSupport patients, families, and referral partners throughout the referral and admission process/liliProvide hospice informational / goals of care conversations with patients and families/liliFacilitate initial patient consent process/liliAttend weekly meetings to report successes, share competitor insights, and identify growth opportunities/liliMeet or exceed performance expectations set by the Director of Business Development and Executive Director leadership/li/ulh3Compensation Benefits/h3pWe offer a competitive salary and lucrative bonus structure:/pulliSalaried position with performance-based bonus potential/liliStarting salary at $100,000+, based on experience/liliMedical, Dental, Vision, and Chiropractic insurance/lili401(k) with company matching/li/ulpIf you are passionate about hospice care, relationship-building, and making a meaningful impact in your community, we would love to hear from you./p/div
