Work Environment\n

The work environment is primarily remote, allowing you to interact with clients in their own homes or within hospice facilities. You will engage with a diverse client base, taking advantage of modern communication tools and electronic health record (EHR) systems for case management and documentation. Full-time working hours are Monday through Friday from 8:00 AM to 4:30 PM, though flexible scheduling may be required to meet client needs, including potential evening or weekend visits. An essential component of this role is your on-call availability to handle urgent situations as they arise.

Key Responsibilities\n

• Conduct comprehensive assessments of clients’ psychosocial needs, evaluating emotional, mental, and social health factors.

Qualifications\n

Compensation and Benefits\n
