Skilled Nursing - Licensed Vocational Nurse - Part-time

LVN/LPN Location: Lake Isabella, California Schedule: 7:00PM - 7:00AM x 4 Term: 13-week assignment with the potential for extension Pay Rate: $39 - $42/hr

Astrya Global, a San Diego based medical staffing agency, is hiring a LVN/LPN for a temporary assignment in Lake Isabella, California. These positions are 13-week contracts with the ability to extend.

Responsibilities for a LVN/LPN:

Assess, plan, and implement nursing care of residents within the skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing Must have skilled nursing experience Administer medications following the eight (8) medication rights to reduce the potential for medication errors Ensure continuity of care of residents between shifts by providing direct care and supervising care given by CNAs and supportive staff members Assess and reassess pain utilizing appropriate pain management techniques; educate residents and families regarding pain management Document changes in residents' condition and nursing care on progress notes and care plans Perform waived testing (point-of-care testing) per Clinical Laboratory and resident care unit policies and procedures; interpret results and take appropriate action Meet current documentation standards and policies Notify physician of changes in resident's condition and follow through until appropriate action is taken Note and conduct physician orders in a timely manner Provide thorough reports to oncoming shifts by participating in walking rounds and documenting upcoming needs appropriately Follow through on information provided during shift reports regarding resident or family concerns Implement plan of care for residents based on assessments and goals established by the interdisciplinary care team Treat residents and their families with respect and dignity; identify and address psychosocial, cultural, ethnic, and religious/spiritual needs Supervise other members of the healthcare team as needed to accomplish the established plan of care Demonstrate competence in technical and manual skills according to scope of practice for LVNs Function as a resident advocate by protecting resident rights Interact with residents, families, co-workers, managers, and other departmental personnel in a kind, knowledgeable, and friendly manner Consistently involve residents and families in planning for care, treatment, teaching needs, and discharge plans Function as a leader for CNAs; delegate in a manner that contributes to the excellence of the nursing unit Participate in orientation of new personnel and assist in performance evaluations when needed Follow proper infection control procedures and supervise supportive staff in infection control procedures Demonstrate the ability to remain calm in emergencies and/or stressful situations Participate in resident and family teaching Maintain current California Licensed Vocational Nurse licensure Maintain current BLS certification Possess basic computer knowledge and Electronic Health Record experience preferred
