Shift Details:

• Night Shift part-time Wed night and Saturday night

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• Assesses residents' conditions and nursing needs and takes appropriate nursing action.

• Assesses and reassesses pain and applies appropriate pain management techniques.

• Educates residents and families on pain management and care needs.

• Documents changes in residents' conditions, nursing care, and care plans accurately.

• Performs and interprets point-of-care (waived) testing and takes appropriate action based on results.

• Administers medications safely following the eight medication rights and established protocols.

• Maintains accurate documentation in accordance with current standards and policies.

• Notifies physicians of changes in residents' conditions and follows through on necessary actions.

• Reviews and implements physician orders promptly and accurately.

• Provides thorough shift reports and communicates upcoming resident care needs.

• Follows up on concerns reported by residents, families, or staff.

• Implements individualized plans of care based on assessments and interdisciplinary team goals.

• Treats residents and families with respect, dignity, and cultural sensitivity.

• Addresses residents' psychosocial, cultural, ethnic, and spiritual needs.

Minimum Qualifications:

• Current, valid, and unrestrictedCA license/certification in good standing, as required for the position.

• Minimum of one (1) year/sof recent experience as an LPN

• MUST HAVE SKILLED NURSING EXPERIENCE

• A degree from an accredited school of Nursing

• Active BLS

• Copy of current, completedPrimary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.

• Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
