Position Summary


The Treatment Coordinator provides leadership in partnering with families and appropriate others in the community to assure that the therapeutic needs of a designated caseload of children are adequately addressed. In each case, the Treatment Coordinator will assure that the services delivered both to the target child/adolescent and to the family are individualized, appropriate to need, child centered, family focused, culturally competent, strengths based, and goal driven.

Essential Roles and Responsibilities

Clinical and Support

1. Assesses and identifies the needs of the birth family to achieve their family service plan goals.
2. Empowers birth families through systems education and advocacy training.
3. Involves the biological family in all aspects of treatment planning including regular updates on youth’s progress.
4. Coordinates regular contact between the child and his/her biological family as appropriate.
5. Works in collaboration with other service providers to develop and implement intervention procedures in the homes working toward achievement of the permanency plan coordinate all community resources pertaining to discharge placement
   arrange aftercare follow up if indicated.
6. Maintains regular contact and open lines of communication with youth’s treatment team and any other significant community persons.
7. Represents agency regarding court involvement of the youth.
8. Maintains documentation of in-home and other ongoing training activities.
9. Prepares written summaries, Individual Service Plans and quarterly updates, discharge summaries, and program contract regarding youth and families.
10. Assists and supports in training activities for TFC parents.
11. Directly supervises all activities of treatment parents related to the treatment and care of TFC youth through regular home visits and phone calls.
12. Provides in-home training as needed for TFC parents.
13. Completes a formal TFC Parent Evaluation including creation and monitoring of a professional development plan for each family.
14. Provides 24-hour emergency “on-call” service to TFC families according to designated schedule.
15. Assists in the arrangement of respite services.
16. Develops each youth’s treatment team including formal and informal supports as identified by the youth, family, and guardian.
17. Links youth and family to appropriate pro-social activities that foster success and healthy development.
18. Demonstrates cultural sensitivity.
19. Responds to all complaints/concerns from TFC parents in a timely supportive manner.
20. Assesses each child’s needs in all life domains and develop measurable and objective treatment plan goals.
21. Provides ongoing advocacy and support for the child’s educational needs.
22. Initiates and coordinates psychiatric, psychological, and other supportive therapeutic services when appropriate.
23. Monitors the progress of the child’s goals and modify the treatment plan as needed.
24. Ensures completion of and collect TFC parent documentation, including LODES, Medication Logs, Respite logs, copies of medical information, and any other required documentation.
25. Assures that all policies, procedures and standards relating to child safety and well-being are followed.
26. Makes regular contact with each youth to assess safety and well being.
27. Assures that TFC parents follow procedures for child illness and provision of routine healthcare.
28. Develops an individual safety plan for each child and assist other staff, in crisis intervention to protect youth, staff, and property.
29. When transporting youth, maintain safe driving procedures.
30. Adheres to mandated child abuse reporting laws.
31. Confirms youth have completed an independent living assessment for life skills.
32. Submits assessment to the appropriate individuals for scoring.

Supervision

1. Supervises any program aides that are assigned to the youth to assist in the implementation of the treatment and service plan.
2. Participates in individual and/or group supervision and staff meetings

Leadership

1. Provides overall leadership and coordination of the treatment team.

Professional Development

1. Attends and participates in all required and other professional trainings.
2. Seeks out resources and learning opportunities to foster ongoing professional growth, independently.

Qualifications

Education/Credentials/Licensure:

1. 1. Valid state driver’s license, safe driving record, and current auto insurance.
   2. Bachelor's degree in human services or related field.
   3. Two (2) years of supervised experience with children and/or families.

Clearances:

1. 1. State Police
   2. FBI
   3. Child Abuse Clearances
   4. CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training per state regulation.

Working Conditions

Physical Demands:

1. 1. This position requires operating a keypad device 25% of the day and electronic/office equipment 25% of the day. Travel is required approximately 5 times per month and works in weather conditions about 25% of the day. Travel includes transporting students to and from school, school-based appointments, and/or field trips. Fingers, thumbs, hands, wrists, elbows, legs and feet are all routinely used throughout the day. The lower and upper torso is used 75% of the day. Employee must meet minimum requirements for hearing, speech, and vision. Walking is required 50% of the day. Standing occurs 25% of the day. Sitting happens 25% of the day. Reaching above the shoulder as well as bending and twisting are performed 25% of the day. This function routinely lifts 15 pounds, 4 times per day. The maximum lift is 50 pounds, 1 time per day. Mental Demands:

1. 1. Continually handles distractions that interfere. Frequently performs multiple tasks, make quick decisions, and concentrate. Occasionally handles diversity of problems.

Working Hours:

1. 1. Non-traditional schedule/ hours. Flexibility to meet children/ family and program needs.
   2. Participates in on-call rotation.
