About the Position

License: Active nursing license must be in hand at submission. Certifications: Basic Life Support certification required. Must-Have: - Acute hospital experience required. - Experience in ICU, MS, PICU, SDU, PCU, IMC, and Obs specialties required. - Previous charge experience required. - Experience with case management and utilization review required. - Experience with discharge planning, needs assessment, ordering DME, and plan of care required. - Familiarity with regulatory compliance, including CMS. Description: The position requires a Registered Nurse to serve as a Case Manager. The nurse will work directly with patients to ensure their needs are assessed and met. Responsibilities include discharge planning and creating comprehensive care plans. Knowledge of Epic electronic medical record system is preferred. Candidates must be flexible with scheduling. The role includes a zero-tolerance policy for tardiness and unexcused absences. All candidates must pass a compliance EKG exam. The position may require floating to different units as needed. Compliance deadlines must be met for completion. The nurse will work collaboratively within an interdisciplinary team. Strong communication skills are essential for successful patient advocacy.
