Position Overview: We are seeking a Full‑Time Nurse Practitioner (NP) to join our dynamic and supportive team. Key Responsibilities: Perform patient assessments, physical exams, and diagnostic evaluations. Diagnose and treat acute and chronic health conditions. Prescribe medications and manage treatment plans. Educate patients on health promotion and disease prevention. Collaborate with physicians, behavioral health providers, and care coordinators. Participate in chronic care management and MAT services as appropriate. Maintain accurate and timely documentation in the electronic health record (EHR). Engage in ongoing quality improvement and team‑based care initiatives. Qualifications: Current and unrestricted Nurse Practitioner license in Arizona, or eligible for licensure in AZ. Board certification as a Family or Adult NP (FNP, ANP, or AGNP). DEA license and prescriptive authority. Minimum of 1–2 years of primary care experience preferred (new grads considered). Experience with MAT, behavioral health, or chronic care is a plus. Strong communication and clinical skills. Proficiency with EHR systems (eClinicals preferred).
