The Stages unit is an acute psychiatric stabilization facility whose goal is to reduce the need for inpatient hospitalization by facilitating step‑down transitions from inpatient care, providing treatment to those in need of involuntary hospitalization, or treating individuals experiencing worsening mental health symptoms that are not yet severe enough to warrant involuntary hospitalization. The integrated treatment team works closely with other Community Reach Center programs and community stakeholders to identify individuals in need and provide individualized and group‑based services, medication monitoring, and care management as clinically indicated. The Case Manager role focuses on assisting persons to develop social, occupational, leisure, and living skills and to support consumers in living successfully in the community, including linking them with resources to obtain and/or learn basic needs. The role also collaborates with the Admission/Discharge Coordinator to obtain insurance authorizations and navigate outside resources. Essential Functions Provide prevention/early intervention, case management, psychoeducation, and other clinical interventions to consumers at Stages Support and coordinate mental health care with the consumer’s clinical care team and primary care provider Facilitate utilization management for insurance authorizations, including organizing re‑auth schedules, compiling notes, submitting to insurance, and coordinating with Medicaid funding sources Attend collaborative treatment team meetings and reviews of consumers Provide crisis de‑escalation when needed Assist eligible consumers in gaining access to needed medical, social, educational, and other services Provide assessment of needs and service plan development Empower consumers to reach their goals and provide role modeling and technical expertise to accomplish goals Provide transportation for consumers in agency vehicles when needed Work closely with outside organizations on resources and care coordination Track success for each individual consumer using an evidence‑based measurement‑based care model Qualifications Bachelor’s degree in Human Services or related field Strong communication, organization, time‑management and clinical skills Ability to work independently in a community‑based setting Valid Colorado driver’s license with a clean driving record Bilingual Spanish preferred but not required Physical Requirements Stand, sit, talk, hear, and use hands and fingers to operate a computer and keyboard regularly Close vision required for computer work Light to moderate lifting required Schedule Monday – Friday, 40 hours per week at Stages. Responsibilities include meeting with clients in a locked setting to assess incoming needs, discuss and coordinate discharge/aftercare planning, set up resources, refer to external agencies/services, and provide follow‑up as needed for continuity of care.
