About Us

Lana'i Community Health Center (LCHC), located on the island of Lana'i, is deeply rooted in a unique history of transitions and cultural diversity. This island has undergone economic and lifestyle shifts driven by changing ownership over the years. These transitions have shaped the island's residents, creating a community of great cultural richness, with a significant population of immigrants.

LCHC serves the entire population of Lana'i, with a special emphasis on those living at or below 200% of the federal poverty level and the underinsured or uninsured. The center provides services in a culturally sensitive manner, offering translation services when needed, and ensures that no one is denied care due to an inability to pay.

Located centrally in Lana'i City, at 333 Sixth St., LCHC is a 501(c)(3) nonprofit organization, ensuring that comprehensive healthcare is accessible to all residents, reflecting the island's unique history and diversity.

As a key contributor to the Lanai Community Health Center, the incumbent in this role plays a pivotal part in ensuring the effective operation of the organization. As a Referral Specialists, you will work closely with Lana'i Community Health Center (LCHC) patients, families, clinical staff, and external organizations to ensure quality patient care by assisting with patients' referrals, coordinating services and resources in regard to referrals, obtaining referrals notes, tracking hospitalizations/imaging reports/procedure/specialist notes, and monitoring follow-through of referrals for patients and the LCHC medical providers. LCHC is a patient care, teaching and research organization and provides workshops, education sessions and outreach - all employees should expect to be involved with these activities to the extent to which will depend upon their position in the organization.

• High school diploma or equivalent required
• Associates degree in social services or equivalent work experience (preferred)
• Demonstrated knowledge of community resources, health services, and social services available to Lanai and the State of Hawaii.
• Productive understanding regarding eligibility requirements, sliding scales, and social programs.

• Full-time availability with a willingness to adapt to the dynamic needs of the organization.
• Commitment to active participation in workshops, education sessions, and outreach activities.
• Adherence to LCHC policies and procedures.
• Dedication to maintaining patient confidentiality and compliance with healthcare regulations.
• Eagerness to contribute to the teaching and research aspects of LCHC's mission.
