Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• 1 year Home Health PT experience
• Valid driver's license and car insurance
• COVID vaccination required

• 8 hr days
• Coverage area includes Mokena and Frankfort

Location Highlights

New Lenox offers convenient access to both outdoor recreation and the Chicago metro area. Hickory Creek Preserve is located in New Lenox and provides miles of trails and forest preserve access, while Downtown Frankfort is about 15 minutes away with dining and seasonal community events. Silver Cross Hospital's surrounding service area includes Mokena and Frankfort, and the location is roughly 45 miles from downtown Chicago for museums, lakefront attractions, and major league sports.
