Key Responsibilities\n

• Conduct comprehensive psychological evaluations and assessments, adapting treatment plans based on each client’s unique needs and progress.

• Facilitate open lines of communication among multidisciplinary team members, families, and caregivers, providing updates on patient development and therapeutic strategies.

• Document all client interactions thoroughly through electronic medical records (EMR) systems to maintain up-to-date and accurate patient charts.

• Implement and monitor individualized care plans, which may include in-home observations or assessments when necessary, ensuring that each client receives personalized attention.

• Participate actively in Interdisciplinary Team Meetings to contribute insights and collaborate on care strategies, enhancing the collective efficacy of our therapeutic offerings.

• Prepare and deliver monthly and quarterly progress reports, showcasing patient milestones and treatment outcomes to stakeholders.

• Conduct training sessions aimed at enhancing team competency in program implementation and effective documentation practices.

• Offer continued support through follow-up interviews, client check-ins, and home visits to reinforce therapeutic goals.

• Develop new training materials and resources to optimize the effectiveness of our counseling programs.

Patient Population / Client Focus\n

You will provide counseling services to a diverse demographic of clients, including both adults and children facing various challenges such as anxiety, depression, trauma, family dynamics, and substance abuse. Your role will encompass not only individual therapy but may also involve family sessions or couples therapy, ensuring that the needs of every client are comprehensively supported.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a closely related field is required.

• Possession of a current and unrestricted state license as a Professional Counselor (LPC).

• Minimum of two years of directly related counseling experience, particularly within a clinical setting.
