Job Title: General | Physical Therapist Location: Chapmanville, WV Setting: Home Health Pay Range: 2614.60 - 2745.33 Schedule: Discuss with recruiter Contract Length: 13 weeks Candidate Type: Traveler Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting; Previous home care experience Responsibilities:

• Perform clinical assessment to identify patient needs and appropriateness of patient care.
• Document, prepare, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation.
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.
• Assist and instruct patients and families in transfer techniques, use of equipment, and therapeutic procedures.
• Collaborate with interdisciplinary team to develop and evaluate patient care plans.
